Please conform BOTH COPIES of each Exhibit, first pages only.
File one conformed copy of Exhibits in/for the record case 10 2 00245-8.
Return the second conformed copy of Exhibits D-G to herein Third Party witness in supplied SASE with conformed Enclosure Letter.
Additionally as Clerk of the court provide the following requested completed information in the SASE;
1. All CUSIP numbers assigned to this and any subsequent case thereof. Include a signed certified true copy thereof with all signatures completed on each/all document.
2. Certified true copies of all signed and completed public debt forms 1522 or otherwise, including any other assigned numbers thereof to this matter, include all court clerk, administrator and contractor signatures on any/all IRS Public Debt US Treasury documents with case number 10 2 00245-8 or subsequent/variations of numbers thereof.
3. Please provide name and signature of designated custodian responsible for “Pooled” monies purchasing Treasury Securities held at the Federal Reserve Bank of Dallas, for Court Registry Investment System (CRIS), pursuant to Paragraph 2.2 “Exhibit F” and Paragraph “B” “Exhibit G”.
4. Provide any other completed forms including any/all Federal Reserve Banks, names of any/all other Custodians, Clerks, and Judge's signatures authorizing CRIS transactions.
5. Please also provide the party's name and contact information, EIN, TIN or other identifying evidence as responsible party to pay all taxes on said public debt form 1522, or substituted form thereof, for any CRIS investments made by ADVANTIS, Pacific County Officials, Court Clerk, Court Administrator or any other party to this matter.
As Third Party Witness, use of name, address, or any identifying information, variation or continued falsification thereof is STRICTLY FORBIDDEN.
