I'd like to emphasize the point of the " ineptness" that exist in the many levels of the seats of leadership within all the governmental entities. This link that I'm providing serves as the epitome... which represent those who ought to be capable of making common sense decisions with respect to holding "someone accountable" when rules are broken...
The VA's response to who and how the medical records got destroyed is the exact type of response that every individual governmental agency provides when posed the questions regarding equal pay, sexual harassment, racism, cronyism, nepotism and so much more.

Within every agency of the government... there isn't any leadership and without leadership, there isn't any semblance of accountability, thereby ensuring that the chain fails because every link in the chain is broken.
This exact concern has gone on for 50 plus years and until those in the seats of power are removed, expect the same.
The reality is, a failed leader can't screw up if they don't have any workers, thus, the power of change exist in those who serve under those who lead and by refusing to work for an entity that allows piss poor leadership isn't worth ones conscientious peace.
